All of the below resources were found in a file created by Autism Speaks:
The file contains information on transition services specific to the state of FL starting at age 14. I urge anyone in need of information on the topic of transition to utilize this source. It starts off by giving you a timeline from beginning to end of an appropriate transition to postsecondary education by age. They cover topics such as starting the process, education, guardianship/conservatorship, vocational rehab, social security, and even housing. I cant say enough good things about Autism Speaks, they have truly made a world of difference for those with Autism Spectrum disorder. The link to this particular file is listed here.

Vocational Rehab or VR is devoted to helping those with disabilities find or keep a job. This is an amazing program here in FL and they are rolling out new programs all the time. Check out this short video about their latest program, STAR.

Project 10 is a critical part of transition to postsecondary education for students with IEP's or 504 plans because they serve as the go between from the Bureau of Exceptional Education and Student Services and the school districts themselves. They work on "addressing law and policy, effective practices, and research-based interventions in the area of transition services for youth with disabilities." The Agency for Persons With Disabilities is comparable to Vocational Rehab. Their mission is to help those with developmental disabilities successfully "live, learn and work in their communities". Before 2004 the agency was linked to the Department of Children and Families.
